The fully complete Panchakarma treatment involves three major phases, each with specific steps and treatments:

1. Purvakarma (Preparatory Phase)

Duration: 5–7 days.

This phase prepares the body and mind for the main Panchakarma therapies by loosening and mobilizing toxins.

Steps and Treatments:

a. Deepana and Pachana (Digestive Stimulation)

  • Purpose: To kindle the digestive fire (Agni) and improve metabolism.
  • Treatments:
    • Herbal decoctions and powders like Trikatu or Chitrakadi Vati.
    • Light, digestible diet.

b. Snehana (Oleation Therapy)

  • Purpose: To lubricate and soften tissues, helping mobilize toxins.
  • Types:
    • Internal Snehana: Consumption of medicated ghee (e.g., Ghritam like Mahatikta Ghritam).
    • External Snehana: Full-body massage with medicated oils (e.g., Abhyanga using oils like Dhanvantaram Taila).

2. Pradhanakarma (Main Panchakarma Therapies)

These are the five primary detoxification procedures tailored to the individual’s doshic imbalances.

a. Vamana (Therapeutic Emesis)

  • 1–2 days (includes preparation and execution).
  • Purpose: To expel Kapha toxins from the stomach and chest.
  • Procedure:
    • Pre-treatment with Snehana and Swedana.
    • Administration of emetic herbs (e.g., Madanaphala or licorice).
    • Induced vomiting under supervision.
  • Post-care: Light, calming diet and rest.

b. Virechana (Therapeutic Purgation)

  • Purpose: To eliminate Pitta toxins from the intestines.
  • Procedure:

    • Oral administration of purgative herbs (e.g., Trivrit or castor oil).

c. Basti (Medicated Enema)

  • 7–14 days (as multiple enemas are administered)
  • Purpose: To cleanse and nourish the colon, balancing Vata.
  • Types:
    • Niruha Basti (Cleansing enema): Herbal decoctions.
    • Anuvasana Basti (Oil enema): Medicated oils.

d. Nasya (Nasal Administration)

  • 5–7 days (daily sessions)
  • Purpose: To cleanse and detoxify the nasal passages, head, and neck region.
  • Procedure:
    • Nasal instillation of medicated oils (e.g., Anu Taila).
    • Accompanied by facial massage and steam inhalation.

e. Raktamokshana (Bloodletting Therapy)

  • 1–2 days (if prescribed, and not always included for all patients).
  • Purpose: To remove impure blood and detoxify the circulatory system.

    • Leech Therapy (Jalaukavacharana): For localized detox.
    • Venous Puncture: For systemic detox.
